Accident Report

An Accident Report should be completed and filed if any employee is working at the time of injury and on the Cornell payroll. Please click on the link for the Instructions for Completing an Accident Report and the Form.

Those individuals covered include:

• Regular Full-Time Employees
• Regular Part-Time Employees
• Academic Employees (all Faculty, including Visitors)
• Temporary Employees
• Student Employees

Please notify the Administrative Service Center, 235 Sage Hall, that an Accident Report has been submitted at AdministrativeServices@johnson.cornell.edu
